It’s time to register for your Yearly Wellness Screening and Flu Shot! This year, the Wellness Committee will award prizes to the Top 3 Buildings, based on participation. You can register online for your Wellness Screening and Flu shot at https://empower.health/decatursd61. 

• Returning Participants: If you previously created an account, sign in under "Welcome Back!
• New Participants: Complete "New User" fields and enter client code: decatursd61 

Employees covered by the District’s health insurance who participate in the Wellness Screening will receive a one-time refund of $100 (for those on individual coverage) or $200 (for all family tiers of coverage) for their participation, if 50% minimum participation is met by the District.
